Grant to a USA and territories nonprofit organization, government agency, IHE, school district, or Native American tribal organization to improve clinical care delivery to sickle cell disease patients. Applicants are advised that required registrations may take up to a month to complete.

OMH is dedicated to improving the health of racial and ethnic minority populations through the development of health policies and programs that will help eliminate health disparities. Through its demonstration grants, OMH supports the identification of effective approaches for improving health outcomes with the ultimate goal of promoting the dissemination and sustainability of these approaches. This particular grant aligns with the following priorities of the Assistant Secretary for Health: Sickle Cell Disease; Health Transformation; Health Opportunity; Health Innovation and the following HHS Strategic Goals:  Strategic Goal 1: Reform, Strengthen, and Modernize the Nation's Healthcare System; Strategic Goal 2: Protect the Health of Americans Where They Live, Learn, Work, and Play; and Strategic Goal 4: Foster Sound, Sustained Advances in the Sciences.2 This initiative is intended to ultimately improve the lives of patients with SCD, most of whom are racial/ethnic minorities, by improving clinical care delivery and thus aligns directly with OMH’s mission.

The Sickle Cell Disease Clinical Data Collection Platform initiative is designed to determine whether a standardized clinical data collection platform, shared across medical centers and other healthcare facilities, can serve as a central repository for analyzing data from large patient cohorts, recruiting patients for clinical trials, assessing adherence to evidence-based clinical guidelines, and identifying new areas for research.  OMH expects the successful applicant to plan, design and develop the data collection platform; develop a plan to align and integrate the platform with clinical care sites; define criteria for participating pilot sites; identify and recruit hub and satellite sites to pilot the shared platform; create and manage a collaborative learning community; evaluate utilization of the data collection platform; and develop dissemination and sustainability plans. When fully developed, OMH expects that the platform will be made widely available to healthcare providers who treat patients with SCD and researchers.

OMH anticipates the availability of approximately $1,000,000 annually to support one award for up to two years.

Award Ceiling (Federal Funds including indirect costs): $1,000,000 per budget period
Award Floor (Federal Funds including indirect costs): $750,000 per budget period

Term of Contract:

- Anticipated Start Date: September 30, 2020
- Estimated Period of Performance: Not to exceed 2 years
- Anticipated Initial Budget Period Length: 12 months

Awards will be funded in annual increments (budget periods) and generally for a project period up to 2 years, although shorter project periods may be approved. Budget periods may also vary from the estimate indicated below due to timing of award issuance or other administrative factors. Funding for all approved budget periods beyond the first is generally level with the initial award amount and is contingent upon the availability of funds, satisfactory progress of the project, adequate stewardship of Federal funds, and the best interests of the Government.

The HHS/OASH GAM will only accept applications via alternate methods (hardcopy paper via U.S. mail or other provider or PDF via email) from applicants obtaining prior written approval. If you receive an exemption, you must still submit your application by the deadline.

As soon as possible, you should discuss the project with the State Single Point of Contact (SPOC) for the State in which your organization is located. For more information, see the SPOC List in Supporting Documents below.

Required Registrations:
- To ensure adequate time to submit your application successfully, HHS/OASH recommends that you register as early as possible in Grants.gov since the registration process can take up to one month.
- You are required to provide a Dun and Bradstreet (D&B) Data Universal Numbering System (DUNS) number when applying for Federal awards through Grants.gov.
- Your organization must register online in the System for Award Management (SAM). Because of the public health emergency with COVID19, the requirement for SAM registration at the time of submission has been waived. Grants.gov will temporarily allow submissions without a current SAM registration. A completed registration in SAM may be required prior to receiving any award based on your application. You should allow a minimum of five days to complete an initial SAM registration. Allow up to 10 business days after you submit your registration for it to be active in SAM. This timeframe may be longer if SAM flags the information you provide for manual validation.
