Grants to USA nonprofit, faith-based, and tribal organizations, agencies, hospitals, and IHEs to improve the lives of older people and those with disabilities by preventing falls. Funding is intended to address falls using a a multi-faceted approach and a comprehensive strategy, including a combination of clinical and community-based interventions. The purpose of the program is to bring to scale and sustain evidence-based falls prevention programs that have been proven to reduce falls, fear of falling, and fall-related injuries in older adults.

This funding opportunity has two goals:

  • Goal 1: Strengthen integrated state or regional networks that address the social and behavioral determinants of health of older adults and adults with disabilities.
  • Goal 2: Significantly increase the number of older adults and adults with disabilities who participate in evidence-based community programs to reduce falls and falls risk.

Networks should have:

  • The capacity to strategically incorporate evidence-based falls prevention programs within a broader integrated health context that includes other programs/services related to the social determinants of health (e.g. nutrition, transportation, care coordination, etc.);
  • The ability to deliver and embed evidence-based falls prevention programs into an integrated, sustainable program network.

Applicants are expected to begin delivering their proposed falls prevention programs within three to six months after receiving their Notice of Award.

  • County governments
  • Independent school districts
  • Native American tribal governments (Federally recognized)
  • Native American tribal organizations (other than Federally recognized tribal governments)
  • Nonprofits having a 501(c)(3) status with the IRS, other than institutions of higher education
  • Private institutions of higher education
  • Public and State controlled institutions of higher education
  • Public housing authorities

  • State governments

Additional Eligibility Criteria:

Eligible applicants are domestic public or private non-profit entities including state and local governments, Indian tribal governments and organizations (American Indian/Alaskan Native/Native American), faith-based organizations, community-based organizations, hospitals, and institutions of higher education.

Foreign entities are not eligible to compete for, or receive, awards made under this announcement.
